General Information
Medication:
**If it is necessary for your child to take medication during school hours it is required that you have your child's Primary Care Provider to complete a Permission for School Administration of Prescription Medication form. You may obtain this form from the school nurse.
** Medication must be in a current prescription bottle.
** Non-prescription medication can be given for no more than 7 days, after that a prescription is required. You may complete a Permission for School Administration of Non-Prescription Medication form for cases when medication needs to be given for a short period of time. You may obtain this for form the school nurse.
Lice:
Students that are sent home due to Lice are not allowed back in school until there is evidence of treatment and there is no live lice and/or nits in the students hair. Checks will be made by the nurse.
Fever:
Students must remain home if they are sick. Students with fevers of 100.4 or greater will be sent home. Students are not allowed back to school until they are fever free for 24 hours, without taking fever reducing medication.
Sick Students:
If your child is not feeling well please do not send them to school. If you do, please be prepared to come and pick them up. We will not allow a sick student to remain in school.
Immunization Records:
All students must have a current South Caroline Immunization Record DHEC-4024 on file with the school or your child will not be allowed to attend school.
Contact Information:
Please update your contact information so that you may be contacted in the event I need to reach you for any reason. I will only call you if I need you to come and pick up your child, if I need to give them medication ( so sign the medication sheets on line) or if it is an urgent matter.
